Publication of Information pursuant to Section 9(2) of Act No. 171/2023 Coll., on Whistleblower Protection
The obligated entity, CYMEDICA, spol. s r.o., Company ID No. 616 82 535, with its registered office at Pod Nádražím 308/24, 268 01 Hořovice, as the obligated entity (hereinafter referred to as the “obligated entity”) within the meaning of Act No. 171/2023 Coll., on Whistleblower Protection (hereinafter referred to as the “WPA”), hereby publishes, in accordance with Section 9(2) of the WPA, the following information in a manner enabling remote access:
1. A natural person who has made a notification (hereinafter referred to as the “whistleblower”) may submit the notification via the internal reporting system of the obligated entity in writing, orally, or in person upon request. A whistleblower may not be a natural person who does not perform the type of work listed in point 4 below for the obligated entity. The notification must be submitted to one of the designated persons of the obligated entity listed in point 3 below.

3. The obligated entity excludes the acceptance of notifications from persons (whistleblowers) who do not perform work or other similar activities for the obligated entity as defined in Section 2(3)(a), (b), (h), or (i) of the WPA, i.e., do not perform such work as:
- dependent work carried out under a basic employment relationship (employment contract, agreement on work activity, agreement on work performance),
- voluntary activity,
- internship or traineeship.
4. The whistleblower may also submit a notification to the Ministry of Justice of the Czech Republic via the external reporting system through remote access at the following web link: https://oznamovatel.justice.cz/chci-podat-oznameni/. Notifications may also be submitted in writing or orally via the contacts below for the Ministry of Justice of the Czech Republic, or in person by prior arrangement (within 14 days of such request) via email at [email protected] or by phone at 221 997 840 (phone calls are not recorded). 5. The whistleblower may freely choose which system to use for submitting a notification, either through the internal reporting system of the obligated entity or through the Ministry of Justice of the Czech Republic.
